FORMS:
- BSRHA Housing Application - Mutual Help, Low Rent and Senior Housing.
- BSRHA Release of Information - Must be submitted with the Housing Application, and Annual Re-certification.
- 4506-T Form - Allows the Applicant, Homebuyer, or Tenant to request copies of their tax returns from the IRS. It will take up to 45 days for the IRS to respond to your request. Your application will not be considered complete without this information.
- BSRHA Involuntary Displaced - Must be completed by certifying agency such as a Tribal Council, City Office, or Public Safety and submitted with the Housing Application.
- Annual Re-certification Update - Used to update information for current programs participants or wait list applicants.
- Things You Should Know - This form is required with completed Housing Application. Please Read Carefully.
- Resident Rights - Tenant information for Senior and Nome Low Rent Tenants.
- Change of Successorship - Homebuyer designates a change of successor to their MHOA unit.
- Alaska Permanent Fund Dividend AOR form - Tenant may assign their APFD to BSRHA.
